Of the 72 patients for whom clinical data was available, 86% presented with clinical symptoms, most of which were suggestive of locally advanced prostate cancer (i.e., obstructive or irritative urinary symptoms, hematuria and\/or hydronephrosis). In 10 cases, the secondary tumor was diagnosed incidentally. Secondary prostatic involvement was due to direct invasion and metastasis (42% and 47%, respectively), the latter being metachronous in the vast majority of cases (95%). The most common site of origin was the gastrointestinal tract (62%), with direct invasion being the favorite route in these cases, whereas lung carcinomas accounted for the majority of tumors involving the prostate by metastatic spread. Most tumors were epithelial (82%, including neuroendocrine neoplasms); of the remaining 15 cases, there were 6 germ cell tumors, 6 melanomas, and 3 sarcomas.<\/p>\n<p>&nbsp;<\/p>\n<p><u>Comment<\/u><\/p>\n<p>Metastatic tumors are grouped together in the same chapter of the current WHO Classification of genitourinary tumors, 5th Ed. Interestingly, secondary tumors to the kidney, bladder and testis are dealt with in separate sections, whereas those involving the prostate and penis are addressed in a few paragraphs within the Introduction section, probably due to their low frequencies. The majority of secondary prostatic malignancies are of urothelial origin, often by direct spread from the bladder. As highlighted by the authors of this study, the most common histotype of such metastatic tumors is adenocarcinoma, often of gastrointestinal or lung origin, with peculiar morphologic features allowing its distinction from conventional prostatic acinar adenocarcinoma, thus prompting adequate immunohistochemical workup, based on clinical and anamnestic data. Nevertheless, the differential diagnosis with unusual histological patterns and subtypes of primary prostatic acinar adenocarcinoma (such as, mucinous and signet-ring cell-like) as well as ductal adenocarcinoma, may be challenging.
